This form reserves one place in the travel group of the Maine Conference Youth Trip to the National Youth Event, Knoxville, Tennessee, July 23-29, 2008.

I acknowledge that reservations for the trip to the National Youth Event are on a first come, first served basis, and that until I receive a confirmation letter I cannot assume that I have a place. I realize that submission of this form stands as a commitment to take part in the above trip.

Transportation for the trip will be by bus. All lodging is included in the trip price. Meals at the National Youth Event are also covered. Snacks and meals while traveling by bus to and from Knoxville, Tennessee will be the responsibility of each participant.

A $100 non-refundable deposit must accompany this form, which will be credited to the total price of the trip.

There will be a mandatory meeting this spring for all travelers, date to be announced.
